Cain is one of his parents’ three offspring. There are two siblings in his family: Efrain Jr. and Adela. Cain and his family were raised in Yuma, Arizona. He won the 5A Arizona Wrestling Championship twice during his senior year of high school. He also played linebacker in high school football.

He captained the wrestling and football teams during his senior year of high school. Cain enrolled at Iowa Central Community College for the 2001-2002 wrestling season. After graduating high school, he wrestled for one season at Iowa Central Community College, winning the 2001-2002 NJCAA National Championship in the heavyweight division. Velasquez then traveled to Arizona State University.

He wrestled for the Sun Devils for three years, compiling an 86–17 record and placing fifth and fourth in the nation in 2005 and 2006, respectively. At ASU, Velasquez wrestled prospective UFC competitors Ryan Bader and C.B. Dollaway. In 2006, he received a degree in interdisciplinary studies from ASU.

Velasquez began training at the American Kickboxing Academy after graduating. In his mixed martial arts début in 2006, he defeated Essa Fujarczyk via first-round TKO. Velasquez then faced Jeremiah Constant, who at the time was unbeatable.

He won by first-round technical knockout. At UFC 83 in 2008, he competed for the first time against Brad Morris. In the first round, he knocked out his opponent. The following year, he fought Jake O’Brien at UFC Fight Night 14.

Velasquez won the first round by TKO. This victory earned Velasquez a rematch with current UFC Heavyweight Champion Brock Lesnar. During that encounter, he came close to tearing his rotator cuff. WWE dismissed Velasquez for “backstage blunders,” thereby exposing the lineup for an impending first. Before signing his WWE contract, Velasquez was also accused of being aware of a knee issue that required surgery.
